help with name

3 pictures of this plant. I've had this plant for..not sure..I'll guess 15 years. I've went through divorce and moved and moved...a few times. This plant has come along with me. It's never been repotted. I've put a bit of new soil in. I fertilize it now and then.
I think once upon a time I knew the name of it. The leaves die and new sprouts keep coming out the top.
Now I've moved and hope to be here for some years to come...so I want to give this plant some attention and care but need to find out the name.
