doing a raised bed, do I dig up the grass or just put soil stright in to the bed?

if the bed is deep enough then the soil will kill the grass and you wont have to lift it. I would think at least 6 " of soil as a minimum depth.

Personally I'd still remove the turf.

yes i would do too especially with the weeds in my grass :o( or should that be the grass in the moss and weeds lol

Cut the grass really low then turn the turfs upside down and put them back in and fill up with soil!
