With most varieties, you leave the spent flowerheads on all winter. As new growth breaks in April, cut off any dead or weak wood and the flowerheads, and cut back strong shoots to a healthy bud or leaf. This is certainly what you do with Hydrangea macrophylla varieties - pruning of H. anomala and H. paniculata is a little different.
