Have seen an off shoot at the base of a holly tree in local woodland which has creamy white leaves with no green whatsover. There is no evidence of insect damage. Some of the younger leaves have a pinkish tinge. Is this rare?

not paticularly. but with no green pigment, chlorophyll, that shoot/leaf will be using food from the other leaves.
