We have this problem almost every year with John Downy, well known for it. We use the fruit for Crab apple jelly, scab and all. It makes no difference from what we can see to the jelly at all.
We never spray at all so cannot help you with that. You could try Copper Sulphate (if you can still but it) which is reasonably organic.

Fairly standard for John Downy crab apples. Scab is bad this year. If you do not eat the Crab apples (as in Crab apple Jelly) then you could use a systemic fungicide, next year.
Collect up any fallen fruit and affected leaves and dispose of, rather than composting or leaving on the ground.
