do I need to replace perennials every year

no - perennial means it comes back year after year, but may disappear completely during the winter - some don't, they keep their foliage, but the flowers don't appear till the following year. Annuals need to be replaced yearly.

Short-lived perennials, such as some Rudbeckias, may need to be replaced every 3-4 years. Some perennials, such as mums, need to be divided every 3 years or so to stay healthy. Some perennials, such as Gaura, will stay happily in the same spot for 20 years or more!
