I suspect that you'd be pushing your luck to move a tree of this age - but if you have to, then prepare the planting hole, dig a trench around your tree and then get out as much of the rootball as you can. pull it onto a sack or plastic sheet and replant it immediately. Water it well for its first year.
I hope it survives, though!
