I bought two dwarf lilac trees last year which have now flowered. About half way down the 'trunk' a new stem has appeared with a larger and different leaf. please advise

yes it looks like it is from the root stock.

Could well be a growth from the root stock. Depends on where the Lilac was grafted. Privet is often used as the rootstock. Carefully scrape off the unwanted stem right back to the point of origin.
