google the disease spangles. i think it is a gall caused by sap sucking insects causing damage. the gall is the plants way of protecting itself. abit like a scab from a bite.

You sure it's an elder? Only this sounds like lime nail gall mite, which is described as slender, tubular red projections up to 5mm long on the surface of leaves. Does the description fit? If that's what it's got, no treatment necessary - it doesn't kill the tree. I have seen it on a Plane tree before, so maybe it is that.
