As a complete but enthusiastic amateur. Can someone tell me do suckers on roses always come from underground and not off the main stem?

Also they need to be removed with a little of the old root, or the stem as any part left can and will grow even more vigorously.

wild rose,s and sucker,s have 7 leaves , normal rose,s have 5, to the best of my knlowedge

They always come from below the graft point. That could be slightly above the ground if the rose was not planted deeply. Anything which does come from the roots is a sucker, otherwise you have to trace it back to the point of origin to see if it is wanted or unwanted.
