should I nip off flower heads on my small geraniums

Not necessary if we're talking about hardy geraniums, unless they produce seeds and you don't want seedlings around.

If you mean Pelargoniums, then yes, take off the flowered stem when the petals fall off the flowers. Cut it off (or break it off carefully) down to the base of the stem.
