When and how do I cut back Hydrangeas?

Depends on the variety - if you talking about a mophead or lacecap variety, you cut off any dead bits and the spent flowerheads in about April, when growth begins. If its a paniculata variety, you can cut those right down now and they'll still flower next year. If its a climbing hydrangea, you just tidy it up a bit, trimming growth that's sticking out back to the wall.
