When planting Boston Ivy against a Concrete Wall how far from the base of the wall and how far apart do you plant them to get best results?

I have a Boston Ivy growing against the back wall of my house. It was planted at least 30 years ago. I simply lifted the nearest paver and planted it, so it was about a foot away from the wall. I only planted one, so I can't answer your other query, but it did eventually cover the whole of the back of the house. It took quite a few years, though, it was pretty slow for some years.
