Can someone tell me what this problem is ?

One of my old hardy fuchsias has had this all summer at the top but has bloomed well lower down. This is a bit unusual but none of my other fuchsias are afflicted. Not to worry too much. If the shrub is big enough to stand it you could take most of it out to make it look better. If you have garden minded visitors keep a sample and check if they know what is wrong. After looking at my fuchsia book it could be thrips... Cannot suggest anything as insecticides mentioned are out date.

I have noticed today that an adjacent Lupin has its lower leaves significantly chewed away !

Its hard to tell but could it be a type of leafminer doing the damage? you could try useing bugclear at the start of the season this works very well on viburnham tinus which suffers from shothole caused by the viburnham beetle.
